Community Health Partners (CHP), stands as a beacon of hope and care for the Namulesa and neighboring communities in Jinja, Uganda. Founded by a dedicated team of local healthcare professionals, including nurses, midwives, and social workers, our organization is deeply rooted in the ethos of community service and well-being. Our primary mission is to provide accessible, affordable, and comprehensive healthcare yhservices to vulnerable populations, including children, adolescent girls and young women, pregnant mothers, and youths. Located strategically in Namulesa – Butiki Road, at a distance of 8.5Km from Jinja Main Referral Hospital, our medical center serves as a lifeline to a population of 7,915.7 for those who would otherwise struggle to access essential medical treatments.
With a VISION of Good Health for all, regardless, and MISSION of providing and integrated Inclusive medical services for children, adolescent girls and young women, and pregnant women, CHP key program areas are: Treatment and Diagnosis, Nutritional Support and Community outreach.

Currently we are supporting 232 people with free medical consultation & diagnosis. We are unable to provide free drugs due to inadequate financing. We have a total of 627 beneficiaries among which include 450 children, 120 women and 57 men, all through the CHP local community insurance package (CHP LIP) that includes the following; Deworming services, Treatment of; Malaria, Severe Malaria, Flu, Cough, Acute Diarrhea, Severe Dehydration & Testing for; Malaria, HIV, & Sickle cell diseases.
The AIM of the CHP LIP is decreasing school absenteeism of beneficiary vulnerable school going children and their mothers/guardians respectively. The cost of the CHP LIP is 10 USD per beneficiary per school term (every three months).
B. Financial Plan Goal
To raise an amount of 15,554 USD (57,549,800 UGX) to increase number of program beneficiaries (vulnerable children and single mothers) accessing the CHP Local community health insurance from 232 to 627 between April -December 2024.


C. Strategic Goals
1. Procuring of drugs worth 11,354 USD for 627 beneficiaries between April-December 2024.
2. Procurement and assembling project equipment (4 bicycle ambulances and Project Laptop), worth 1000 USD, to used between April-December 2024.
3. To conduct 36 weekly follow-ups amounting to 920 USD, between April-December 2024.
