I am writing this letter to formally request financial assistance for the medical procedure of my father, Mr. Jessie H. Carino, who is currently an outpatient of Philippine Orthopedic Center.
My father requires a titanium implant, specifically a Distal Femur Lock Plate, along with several specialized screws and surgical supplies. Based on the quotation provided by the supplier, Ti-Tech Medical Trading, the total cost for these implants is Php 199,000.00.
We have exhausted all efforts to gather funds and as a solo parent, I am the sole provider for my child and am also shouldering the daily medical expenses of my father. I do not have the financial capacity to raise the amount needed to finalize the procurement of these implants. Without the full payment or additional cash assistance, the surgery cannot proceed, and my father’s recovery remains on hold.
