I am writing because I am currently experiencing financial hardship, and would like to find a way to get back on my feet. I had 4 major surgeries back to back. Tubes put in my ear, 5 months later Rotator cuff ,2 weeks later brain surgery, 4 months later disc fusion surgery. I did not get the chance to go to physical therapy after rotator surgery, my dad died, 3 rd degree burns on my hand, hvac went out, 2 cousins died, lights/water cut off, then my stepdad died. All this in 10 months.
My problems began when I tried to go back to work. The exact day I was supposed to return I ended up in the emergency room with chronic pain,lacerations, cough, and pink. eye, My job requested a medical certification form be completed by my doctor. My doctor sent a 54 page medical certification with numerous reasonable accommodations requested. My supervisor sent me an email with the results of a high rise desk and that was it. I told him several times that was not correct. He ignored it. I was trying to work onsite but I couldn’t . Falling asleep due to medications, in pain constantly. I did not have the accommodation that I needed and that was requested. I was sent home. I applied for short term disability but was approved June 2022 for preliminary long term disability for a year. I was mailed an approval letter and so was my job in June. For months my job did not acknowledge preliminary long term disability approval. They insisted I do requirements for the short term which is to have my doctor complete 703 form every month. and I struggled to get it done each month. Preliminary I am not required to do anything but submit 703 if I had earnings for that month. Only the Medical review board could request the 703 to be completed by my doctor. I received an approval letter from my job with someone else's name on it and it was approved for short term, not preliminary ;ong term. This happened after approval in JUne. My first check took forever. I kept trying to communicate with emails and calls. No response most of the time or out of the office response email. I contacted my HR rep, her supervisor Ms. Poerterfield and Ms. Porterfield supervisor Roger Allen Who assured me they would work on the issues. I called all three for about 2 weeks and spoke with my HR rep who promised she would work on it the next day. All of them were out of the office for days. I was given promissory letters for court saying I would get paid on this day so my car would not get repo. Didn’t get paid and the director at my primary care doctor out of her own personal pocket cash app me 400.00 so my car wouldn’t get repo. My first check was mailed to the wrong address and I was sent an email. It's my responsibility to update my address in Beacon. WHY???? My address hasn’t changed in over 5 years. Mind you, when I owed them money, a letter came to my house. that they took 1600 out of my check . According to the handbook I was supposed to be sent a certified letter and be allowed to make arrangements. I wasn't. Just out of the blue they took my money in Dec for an overpayment in Oct prior..This began months of unpaid mortgages. I called and was told we aren’t giving you the money back and hung up on me. Another overpayment for 61.00 but that letter came certified. My first check I was told I would get paid and that the information was submitted before payroll cut off.
On payday I didn’t get paid. I called payroll and was told they just didn’t get to it and I had to wait a month until the next pay period. 30 more days they don’t write checks it would mess up payroll. I said some nasty things and told them I filed a complaint with HR, Dept of labor, and called the Governor Roy Cooper office and I was getting a lawyer.. This is when the retaliation started. Week later I was called and said they were writing me a check and mailed it to the wrong address.It took additional weeks to get my check. During that time my water and lights were cut off and I couldn't pay other bills also. Second check came correct. Direct deposit. Third check didn’t come. That day I called my HR rep. She said they overlooked it and she was requesting a paper check to be mailed. All during this time the mortgage was 10 months behind, utilities off, phone off, applied for food stamps, my car broke down and was towed. I sent my HR rep all my past due bills and the information about my car from the towing company and DMV.). And she kept sending me letters to give to the towing company about my payment. By the time I got the check it cost over 5000.00 to get my car out of tow. I had to let it go. I missed numerous doc appts, physical therapy, pain mgt, I couldn’t go get my medications, grocery store, laundry nothing. I lost 32 lbs, my hair fell out bald spots , depressed, couldn’t interact with family or others, eat, cook, get dressed, because I couldn’t attend treatments given or take medications. I advised numerous times to contact NC Retirement about approved preliminary and requirements. I stopped having my doctor complete 703 and my payments were stopped. I would send emails/calls and get no response. I asked specifically to please answer my questions and still nothing. My doctor and I decided I would do a trial work period to see if I could work. I emailed my supervisor In August 2022 to let her know and she said I was not suppose to come back until March 2023 7 months later. On aug 29th 2022 I had the return to work letter but could not because I was in the emergency room with chronic pain,lacerations, cough, and pink. eye, Specific doctor's note was given to me to immediately go see my specialty doctor the next day. . I called the call outline and sent pictures, a doctor's note , and emailed my HR benefit rep. The next morning my HR rep responded to my email because I advised that I would not be returning to work and need to file for an extension for disability and she emailed me the documents I needed to complete and sent a copy of the documents she had completed for my extension. I immediately that same day filled them out and submitted them. While waiting for a response from the Medical review Board .I was terminated Sept 16 2022 for no call no show which is not true. I did not find out I was terminated until November 2 months later. My last payment was February 12 2023 and I have not had any income since.
My life and financial situation has been greatly impacted by my employer wrongful termination, disability discrimination, retaliation, unfair treatment, undue financial hardship, undue emotional /depression, compensatory damages (lost of my car) applied for food stamps, lost my phone services, punitive damages, my income has decreased significantly and for months at a time I didn’t get anything while on disability and I have been unable to pay my bills or get back current in good standings. My credit score went from 700’s to 438. My family and I have been forced to cut expenses wherever possible, but we are still struggling to make ends meet.and I have experienced a hardship that has made my life a living hell and difficult for me,my health, and my family that doesn’t seem like I can recoup from.All stemming from the unfair treatment from my job.
I was diagnosed with Pain syndrome, chronic may keep you out of work for weeks or months. I have also been diagnosed with fibromyalgia, anxiety disorder, Muscle/back spasm, Cervical nerve root disorder, Degeneration of intervertebral disc of cervical, Left cervical radiculopathy, Bursitis of left shoulder, Depression, Fusion of spine of cervical region, Chronic left shoulder pain, Impingement syndrome of shoulder, CSF leak from ear, Thyroid mass, and 3 surgeries back to back.
NC Homeowner assistance Funds did pay the 10 months mortgage behind which ended March 2023 and I have not been able to pay my mortgage yet again for 9 months now behind. My mortgage company just did a loss mitigation for 3 months and then will review my situation. Currently I am doing 90% better health wise but can only work at certain jobs due to my disability. I applied for regular disability but they denied me. I have had several interviews and 2 offers but could not accept them because I have no transportation. One offer said if I could be onsite for 8 week training I could then work from home but I couldn’t even get there to do the training.
But at this time I have exhausted all of our income and resources so we are turning to you for help.I am confident that if I could get a car I can and will get a job and back on my feet with no problem. With my skills, ability, experience, and numerous credentials I will obtain and maintain full time employment and my financial obligations and goals. However, there’s one significant obstacle standing in my way: reliable transportation. I currently do not have access to a car, which makes commuting to potential job opportunities incredibly difficult and time-consuming.This is where I am humbly reaching out for your assistance. I am kindly requesting a car donation to help me overcome this obstacle and reenter the workforce. Your generous donation of a vehicle would not only provide me with the means to seek employment but also the freedom to create a more stable and secure life for my family. I understand that a car donation is a significant and generous contribution. In return, I am committed to working hard, achieving my goals, and giving back to my community as soon as I am back on my feet.
